What is Fast Track
Hospital Fast Track helps hospital teams quickly connect patients to structured outpatient care when PHP or IOP may be appropriate.
When to use Fast Track
Use this pathway when a patient is:
- Discharging from ED or inpatient psychiatric care
- Needs structured outpatient support (PHP/IOP)
- Stable for outpatient group care therapy (not requiring inpatient)

How to Refer

What we need from you:
- Patient or guardian phone number, if applicable
- Referrer name and contact information
- Current setting and discharge timing
- Presenting concern
- Insurance (if available)
Operating hours: Monday – Friday, 8am – 5pm MT
Referrals received after hours will be reviewed the next business day.
Insurance Accepted
Guidelight accepts most commercial insurance plans, along with Medicaid (Colorado Access, RMHP, Denver Health, Northeast Health Partners) and Medicare.

What Happens Next
A Guidelight team member will follow up within 15 minutes.
Evaluation is scheduled to determine level of care/appropriateness of the program.
Referrer receives follow-up and status updates.
