Collaboration is built into the foundation of our programs through multiple points of clinician engagement and regular progress reports pushed to provider practices. This creates opportunities for input in the individualized treatment plan. Collaborative discharge planning is a highlight of this and ensures a successful step down in care that should keep the patient progressing.
Outcomes study conducted by top university
Guidelight recognizes that outcomes are patients’ lives and we treat them accordingly. We have engaged one of the top universities in the country to help us study our outcomes and learn from them as an organization.
